What to Do When You Have a Qualifying Event Such As…..

You Get Married
Add your spouse to your medical, dental and vision plans within 30 days of the date of marriage.
1.    Complete enrollment change forms for Medical Mutual and Guardian
2.    Provide a copy of your marriage certificate
3.    Update your beneficiary information (if applicable)
4.    Submit documentation to Human Resources

You Get Divorced
Remove your spouse from your medical, dental and vision plans within 30 days of the date of divorce.
1.    Complete enrollment change forms for Medical Mutual and Guardian
2.    Provide a copy of the divorce decree
3.    Update your beneficiary information (if applicable)
4.    Submit documentation to Human Resources

Have A Baby or Adopt
Add a child to your medical, dental and vision plans within 30 days of the birth or adoption.
1.    Complete enrollment change forms for Medical Mutual and Guardian
2.    Provide a copy of the birth certificate, birth letter or adoption papers
3.    Update your beneficiary information (if applicable)
4.    Submit documentation to Human Resources
*It is not necessary to wait for the Social Security number to be issued

Your Spouse loses coverage from their employer
1.    Complete enrollment change forms for Medical Mutual and Guardian within 30 days
2.    Provide a letter from your spouse’s employer that states the reason for loss of coverage and the effective date.
3.    Submit documentation to Human Resources

Death of Your Spouse or Dependent
Remove your spouse or dependent from your medical, dental and vision plans within 30 days of the date of death.
1.    Complete enrollment change forms for Medical Mutual and Guardian
2.    Provide a copy of the death certificate
3.    Update your beneficiary information (if applicable)
4.    Submit documentation to Human Resources
